Nicola Sarandria is an Italian physician, researcher, entrepreneur and author[1] active in both scientific and literary fields. His work includes publications in immuno-oncology, healthcare management, and aging, as well as narrative works associated with dark fantasy, fantasy fiction, and experimental writing. In the literary field, he has also published under the pseudonym C. Wain.

Life and education

Nicola Sarandria was born in Padua, Italy. He graduated in Medicine and Surgery and subsequently completed academic and professional programs abroad, including experiences in the United Kingdom, Switzerland, and the United States. He graduated with 110 cum laude from Humanitas University in Milan. He went on by completing his MBA in Healthcare Management from North Wales Management School and a PhD in Switzerland. He also holds a post-graduate diploma in cancer genomics and immuno-oncology from Harvard Medical School.

His professional path has developed around medicine, clinical research, immuno-oncology, and scientific publishing. During his university years, he also became interested in experimental music, composing under the pseudonym Vi-S0. This creative activity stands alongside his scientific and literary output.

He travelled extensively and lived in several countries. From Italy and across Europe to USA, UK, Asia, Russia and Africa.

On his maternal side Sarandria comes from the ancient branches of the Princes of Noia Pignatelli and Folch de Cardona family (South Italian branches - Spanish - Princes of the Sacred Roman Empire).

Career

Sarandria’s career has developed along two main lines: medical research and writing. In the scientific field, he has mainly worked on colorectal carcinoma, rectal cancer, immuno-oncology, and the role of tumor-associated neutrophils. In the literary field, he has published works of fantasy and dark fantasy fiction, combining medical research with an independent narrative production.

Sarandria’s scientific output is mainly connected to immuno-oncology and the study of colorectal carcinoma. In 2022 he published A Literature Review in Immuno-Oncology: Pathophysiological and Clinical Features of Colorectal Cancer and the Role of the Doctor-Patient Interaction in the Journal of Cancer Therapy. The article analyzes the pathophysiological and clinical aspects of colorectal carcinoma and also addresses the role of doctor-patient interaction in the oncological context.

In the same period, Sarandria co-authored Private Medical Departments after COVID-19: A Review and Essay Regarding the Need for Improvement of Medical Facilities and the Need of Proper Healthcare Management, published in the American Journal of Industrial and Business Management. The essay, written with Domenico Sarandria and Marcella Cardone, focuses on healthcare management and the need to improve the organization of medical facilities in the period following the COVID-19 pandemic.

Another relevant contribution is Neutrophils in Rectal Cancer: The Sarandria Score, published in the volume New Horizons in Medicine and Medical Research Vol. 7. Also in 2025, he launched the OR1 - Olimpia Research One project, born from Nicola Sarandria’s determination and commitment to contributing to research on colorectal carcinoma, one of the most common malignant tumors in the world, with the aim of supporting the scientific community and, above all, patients. The project developed through more than one year of research and development. The in-depth analysis conducted within the project led to new discoveries and to the formulation of The Sarandria Theory, which led to an international conference presentation, two scientific articles, and a scientific book dedicated to the study results published by Santelli Editore.

Dr Nicola Sarandria is an entrepreneur and was involved in several projects and ventures, including being a part of a fast-growing private company valued at over 40 M dollars. He has extensive skills as a high-level executive in the med-tech world.

As of 2026 he serves as Healthcare Director for Olimpia Medical Center while being member of the board of directors and Scientific Director of Planet Healthcare. He is also involved in projects in South Korea.

Literary career

Alongside his medical and scientific work, Sarandria has developed a literary output focused mainly on fantasy, dark fantasy, grimdark, and speculative fiction. Under the pseudonym C. Wain, he published A Prayer for the Necromancer, the first volume in The Shinigami Tomes series. The book got awarded the 5 Stars Seal from Readers’ Favorite, became part of the Amazon Bestseller list and was featured in the literary magazine “Kirkus Reviews”. A manga adaptation is set to be released in 2026 in collaboration with the Moroccan manga artist Soufiane Rezzak.

The trilogy also includes Dark Light and Bakemono, later collected in The Shinigami Tomes - Gold Edition: The Complete Trilogy, a work that links the pseudonym C. Wain to the name of Dr. Nicola Sarandria.

Sarandria’s fantasy fiction is characterized by dark atmospheres, supernatural elements, fantastic creatures, and a narrative tone close to grimdark. A Prayer for the Necromancer presents a world populated by ghouls, orcs, elves, goblins, and hybrid figures, introducing characters such as Kaze, described in online literary materials as the first “demighoul.”

In “Rusalka” he was aided in the development of the story by his writer friend, Bram Stoker award winner and co-author with George RR Martin of the story “Warship”, George Guthridge.

In 2023 Sarandria published Corde vocali, issued by Rossini Editore. The book appears in the catalogs of Italian bookstores such as Hoepli, Feltrinelli, IBS, and Libraccio.

Among his most recent works is Goemul e altri racconti, published by Edizioni Clandestine and distributed by Mondadori Store, Librerie.coop, and other retailers. The volume is presented as a collection of stories with speculative and dystopian elements.

Dr Sarandria also wrote and served as author for US Army associated magazines such as “Stars and Stripes” and for NATO-UN-Peacekeeping and anti-terrorism associated magazine of COESPU.

According to the editorial biography published by Santelli Editore, The Shinigami Tomes first volume, A Prayer for the Necromancer, received the 5-stars seal recognition from Readers’ Favorite.

The Sarandria Theory, a medical concept developed by Dr. Nicola Sarandria. It explores how certain immune cells behave in rectal cancer. It focuses on how these cells may cause the tumor to grow and spread.The theory is based on a 2025 research study (the OR1 study) that looked closely at tumor tissue. It focuses on three key parts: 1. Tumor-Associated Neutrophils (TANs) 2. Arginase-1 (ARG1). 3. Perineural Invasion (PNI).
